Eyelid malposition can be caused due to inward (entropion) or outward (ectropion) turning of the eyelids. This can be caused due to a variety of reasons including old age, injury, or rarely as a birth defect. This can cause the eye shape to look unsightly and makes the eyes look uneven. It can also be associated with inward turning of the eyelashes or poor eye closure causing damage to the eye surface and subsequent vision problems. The position of the eyelids contributes to the blink mechanism, affecting the drainage of tears from the eye. Any abnormality in the eyelid position can thus lead to disturbance of the regular tear drainage mechanism causing watering or tearing.
Assessment of eyelid malposition involves a detailed examination of the eyelids, surrounding tissues and of the eye itself as all of these factors contribute in deciding the type of surgery to be done.
There are many types of surgeries designed to treat eyelid malposition and vary depend on the type of problem, the eyelid involved (upper or lower), causative factors and severity. Surgery usually involves either local reconstruction which moves surrounding tissue to correct the defect or using skin grafts to treat tissue loss, or a combination of these two techniques.
